Investigation Summary
Employee #1 was moving material from a rented moving van with a lift gate, as a coworker was operating controls to lift gate. Employee #1 had his toes between lift gate and bumper of moving van. Two toes were crushed with the tip of one of the injured toes being amputated, after the coworker pulled the lift down without looking.
