Investigation Summary
On May 2, 2010, Employee #1, a supervisor of American Residential Service Investment Holdings, was dragging a boxed water heater across the smooth cement floor of a warehouse, walking backwards and gripping two handholds on the top of the box. He lost control of the box and it slipped out of his hands. As Employee #1 was trying to get out of the way of it, it struck his knee and he fell down. The box fell and bounced off of his right ankle, fracturing it. Employee #1 was hospitalized due to his fractures.
