Investigation Summary
On April 2, 2010, an employee was crushed and killed when a free standing concrete block or CMU wall that was approximately 12-in. thick, 12-ft long and 23-ft in height collapsed. The wall was not doweled to the footing via any steel reinforcing materials nor was it braced in any way to prevent it from collapsing. Also, no limited access zone had been established adjacent to the wall. Storms with high winds were noted in the area just prior to collapse. The employee was pronounced dead at the scene.
