Investigation Summary
On March 30, 2010, an employee was installing 2 by 4 boards on a set of scaffold brackets 17-ft above the ground. He was connected at the waist only, using a chain with a safety hook and body harness as a positioning device. He was tied off to a metal brace on an ULMA formwork waler when the brace broke. The employee fell 12 ft 4 in. to the concrete below. The employee died at the scene.
