Investigation Summary
On May 26, 2010, Employee #1 and Employee #2, were assigned the task of removing old flashing and glass windowpanes on a foundry building. The foundry building has a lower roof of approximately 1:12 pitch, a set of vertical facing windows, an upper roof of approximately 6:12 pitch, a set of vertical windows, and then the roof peak area called the doghouse. Both the lower and upper roofs have skylights installed throughout the whole length of the roof areas. Employee #2 was working the upper roof area removing the old flashing and glass windows and installing the new flashing and metal panels. Employee #1 was assigned to stay on the lower roof area removing the old material dropped by Employee #2 and passing up new material, the flashing and metal panels. Employee #2 wore his harness and connected to a retractable lanyard anchored with an anchor strap around one of the main window frame supports. Employee #1 wore his harness and six-foot lanyard, however; he did not have a retractable lanyard to enable himself to be anchored. The skylights on the lower roof did not have covers or guardrails for fall protection since employees on the roof were to use personal fall protection. Employee #1 fell through a skylight to the concrete floor below approximately 27 feet and died.
