Employee Falls in Water and Drowns

On August 8, 2011, Employee #1 was performing maintenance at a hydroelectric plant. He entered the turbine well with a rope tied around his waist and the other end secured to a fixed point. Employee #1 was attempting to place corrugated aluminum sheets in front of a turbine to slow the water flow through the turbine. As he placed the sheets, he slipped into the water. Employee #1 could not extricate himself because of the rapid water current and drowned.
