Employee Amputates Fingers and Thumb in Moulding Machine

On March 23, 2010, Employee #1 completed the setup of a Weinig Unimat Model Number 22 moulding machine for a specified production run. The machine was operating at full speed (6,000 rpm) when Employee #1 realized a wooden shoe guide had not been removed. Employee #1 should have powered off machine before removing wooden shoe guide. Employee #1 removed the wooden shoe guide located between the fence and cutting head number three. Employee #1 sustained partial amputation to his index finger, middle finger, and thumb. Employee #1 was not hospitalized.
