Investigation Summary
On June 25, 2012, an employee was working as a laborer for a school district in California. He was pulling a vacuum scrubber, when he lost his footing. He fell face first to the floor. He sustained a fractured right eye socket, a fractured sinus bone, and a hematoma on his right arm. He was taken to a hospital, where he was admitted for treatment.
