Investigation Summary

At approximately 12:30 p.m. on June 21, 2012, a carpenter, employed by a general building contractor, was hanging sheeting on the exterior of a home addition in Brentwood, CA. He was standing on a ladder placed on a rolling scaffold. He fell 8 feet to the ground. He was transported to John Muir Medical Center in Concord, CA, where he later died. The Brentwood Police Department phoned a report of this incident to Cal/OSHA Concord District Office at 1:45 p.m. on June 21, 2012.
