Investigation Summary

At approximately 11:10 a.m. on September 16, 2011, an employee, employed by Reliable Service Company, Inc., was using a Makita 4-inch angle grinder to remove shelving and metal bracing at a commercial building. He sustained lacerations to his left forearm. The employee was transported to John Muir Hospital in Walnut Creek, CA, where he received several stitches and then was released. The employee suffered no bone loss and was not hospitalized for more than 24 hours. The accident was called in to the Concord Cal/OSHA office on September 17, 2011, at approximately 11:00 a.m. This incident categorized as unreportable.
