Investigation Summary
At approximately 1:00 p.m. on October 31, 2011, Employee #1, a journeyman painter, sustained an amputation to the right ring finger while working at a local refinery. Employee #1 and a coworker had just returned from lunch and were retrieving their tools and personal protective equipment from the tool box, located on the truck. The coworker closed the tool box and it did not latch completely when he closed it, he then reopened to slam it shut, and to latch it. At the same time, Employee #1 saw a garbage bag hanging out and went to push it in the tool box. The coworker did not see Employee #1 reaching into the toolbox, and the box came down and amputated the right ring finger to the first knuckle. Employee #1 was hospitalized for several days following the accident.
