Investigation Summary
On April 26, 2011, Employee #1 was surveying the awning of a roof from a ladder. Employee #1 fell off the ladder, a distance of approximately 12 feet from the awning to the concrete walkway. He sustained a serious injury, apparently involving fractures to his foot and ankle. Employee #1 was transported to the hospital and was hospitalized for treatment of his injuries.
