Investigation Summary
At approximately 8:35 a.m. on February 14, 2011, Employee # 1 was working for his employer Oliver and Company, Inc. a construction management company. Employee # 1 and the project manager were walking behind the job trailer; he tripped over a two (2) inch water line and fell to the ground impacting his right shoulder. As a result, Employee # 1 sustained fractured/dislocated right shoulder from the fall. Employee # 1 was transported and admitted to Concentra Medical Clinic, where he was hospitalized for more than 24 hours. The accident was reported to DOSH (Division of Occupational Safety and Health) on February 14, 2011. .
