Employee Is Struck by Falling Object and Injures Neck

On July 23, 2010, Employee #1, of US A Shade and Fabric Structures, Incorporated, was assigned to remove loose dirt at the bottom of a 15 ft deep pier hole using a five gallon paint bucket. Once the bucket was full, Employee #1 instructed Employee #2, who was above the surface, to pull up the full bucket and empty it. While Employee #2 was pulling up the bucket, the handle of the bucket snapped off, and the bucket fell onto Employee #1. Employee #1 was airlifted to the hospital and treated for a serious neck injury.
