Investigation Summary
At approximately 9:30 am on June 5, 2010, Employee #1 (foreman), and Employee #2, both window cleaners, of Delta Window Cleaning Company, they were tasked to power wash the windows and walls on the outside of the building. They were inside the basket of a Terex TB 85 Aerial Boom Lift. The basket came within a 1 ft. of a 115,000 volt overhead power transmission line. While power washing the windows and walls of the building, the highest point of the man lift came within 1 ft. of the lowest overhead high voltage line, causing an arc flash and creating a fire inside the lift basket. Employee #1 and Employee #2 were engulfed in the flames caused by the arc flash and subsequent fire. Employee #1 suffered burns to 70% of his total body surface area and died of his injuries at 2:30 a.m. on June 23, 2010. Employee #2 suffered burns to 50% of his total body surface area.
