Investigation Summary
On October 18, 2010, Employee #1 was a groundskeeper. He was riding in a golf cart to get another golf cart. When he reached the other golf cart, Employee #1 grabbed a bucket of trash as he stepped out of the cart he was riding in. He lost his balance and struck his head. He died from his injuries which were not specified.
