Investigation Summary
On October 18, 2010, Employee #1 was a groundskeeper. He was riding in a golf cart to get another golf cart. When he reached the other golf cart, Employee #1 grabbed a bucket of trash as he stepped out of the cart he was riding in. He lost his balance and struck his head.
