Investigation Summary
On March 5, 2012 Employee #1 was taking measurements on the second floor of a steam generator plant that was under construction at a new electric power plant. For an unknown reason Employee #1 fell through the grating (one panel had been removed to allow piping to be hoisted to the fourth floor) sixteen feet & nine inches to the ground floor. Employee #1 was wearing a harness with two lanyards but both were attached to the harness at the time of the fall. Employee #1 was unresponsive after the accident and was transported to the local hospital where he died the next day.
