Investigation Summary
On March 2, 2010, Employees #1 through #4 were working on the top of a tank, approximately 40 ft high. An ignition source cause the contents of the tank to catch fire. An explosion occurred followed by a fire. Employees #1 and #2 were killed. Employees #3 and #4 were hospitalized with unspecified injuries.
