Investigation Summary

On February 12, 2010, an employee was installing the last piece of gutter while standing on the third step of the ladder. A coworker was on the roof holding the gutter while the employee was fastening the gutter from the 6-ft A-frame ladder that was positioned on the ground. The employee lost his balance, fell backwards, and hit his head with the ground. The ladder fell towards the North side of the house. According to witnesses, the ladder was in leveled ground and free from defects. The week following the day of the accident, the employee showed up to work complaining about a sore leg. The employee took leave time from February 19 to February 22. While on leave, the employee contacted the company indicating that he had something in his head that he believed was from the fall. On February 24, the wife of the employee contacted the company and indicated that the employee had a seizure. After the seizure, the employee was in state of coma until the family decided to disconnect the life support apparatus on March 4, 2010.
