Employee Is Killed When Conveyor System Activates

On March 26, 2010, Employee #1, an employee of Hermann Brothers Logging and Construction, was sitting on a conveyor. The conveyor system activated and crushed Employee #1. Upon further investigation, it was determined that locks were not used to lockout the conveyor system.
