Investigation Summary
On June 28, 2010, Employee #1 was disassembling the drive frame of a ferris wheel. The wheel became energized and the employee slipped. He was trapped between the wheel frame and the support frame and was impelled by the brace of the drive frame. Employee #1 was killed.
