Investigation Summary
On May 12, 2011, Employee #1 was accessing a fabricated vessel by way of a multi-use scaffolding system. He was welding a tank that was located on the vessel. As Employee #1 began the transition from the scaffolding to the vessel, the scaffolding system became unstable. Employee #1 lost his balance and fell approximately 11.4 feet to a concrete surface, striking his head. Employee #1 was killed from a severe fracture to his head.
