Investigation Summary
On October 10, 2010, Employee #1, #2, #3, and #4 were cleaning the inside of a 75 ft tall by 15 ft wide confined space area within a sulfur plant. While performing the duties, an explosion occurred within the plant and all employees sustained unspecified injuries. Emergency medical personnel were summoned. They arrived and transported all employees to a medical center. At the medical center, Employee #1, #2, #3, and #4 received treatment and were hospitalized.
