Investigation Summary
At approximately 2:24 pm, on May 27, 2010, Employee #1, a Security Officer of Sutters Places, Inc. received a hip fractured after a fall to floor. Employee #1 was arriving to work via the employee entrance double warlock swinging door. Employee #1 opened the door and attempted to step inside the facility, when either the wind cause the door to closed or she tripped and felled causing a fall to the epoxy tile floor. Employee #1 was hospitalized due to her fractures.
