Investigation Summary
On February 19, 2010 Employee #1 was assisting in the loading of a semi truck trailer, being filled by two powered industrial trucks. Employee #1 was opening and closing a key activated overhead door in order to regulate the entrance and exit from the processing area. Employee #1's was very close to the path which must be taken by the powered industrial trucks to load the semi truck. The overhead door was opened by Employee #1 when the industrial truck struck Employee #1. Employee #2's view was partially obstructed by two bales of paper. Employee #1 received blunt trauma to the back of the head and was pronounced dead at the hospital.
