Investigation Summary
The employee, a security system installer, was installing wire about 16 feet above ground level. The employee placed a portable fiberglass 24-foot extension ladder about 14 to 15 feet above ground level. He placed the ladder on the wall against a section of 4 conduit pipes that were fixed vertically down the length of the wall. The conduit pipes were unevenly mounted, with one pipe projecting approximately 0.75 inches to 1 inch from the remainder of the piping. The last rung of the ladder rested on the projecting conduit and the foot pads of the ladder were not firmly planted on the concrete floor, making the ladder unstable. The feet of the ladder slipped backwards, and the ladder fell down the wall. The employee fell with the ladder, struck the floor face first and was killed.
