Investigation Summary
On January 17, 2011, Employee #1, a construction laborer, was part of a six-man crew that was performing roofing work on a commercial building. Three workers were elevated about 22 feet, preparing a 2-in-12 pitch metal roof for weatherproofing application. The metal roof consisted of 22 corrugated flush-mount translucent skylight panels that were spaced about 20 feet apart. The skylight panels were not guarded, and Employee stepped on and fell through the 3 by 10 foot skylight panel to the concrete pad. The worker died on the scene.
