Investigation Summary
At approximately 9:30 a.m. on December 30, 2009, Employee #1 was on the flatbed trailer of an International truck inside a warehouse assisting with loading the second of two slabs of granite weighing approximately 900 lbs each. The granite slab began to tip away from its A-frame support toward the direction of Employee #1. It was not clear as to what caused the slab to slip, but it appeared that Employee #1 stepped backwards to get out of the way. He fell off the flatbed truck and hit the back of his head on a concrete floor, resulting in a skull fracture. He was flown by Life Flight to Metro Health Medical Center in Cleveland, OH. He later died on January 5, 2010.
