Investigation Summary
At 3:45 a.m. on January 30, 2010, Employee #1, a maintenance supervisor, and four other employees of Cargill Meat Solutions began to dismantle storage racks, which were approximately 20 ft to 22 ft tall, from a cooler. The employees used two Genie 1930 scissor lifts to reach the top of the racks. Using straps, the employees lowered the end of the racks, while they were standing approximately 20 ft high in the elevated lift. Employee #1 fell. Emergency services were contacted, and Employee #1 was transported to the hospital, where he later died from internal injuries sustained in the fall. The exact nature of the fall was not specified, but it was believed the cause could be due to improper training on the use of a scissor lift.
