Employee Is Injured in Fall from Lift, Later Dies

On May 13, 2010, Employee #1, a driver of a high lift truck, went up to the top rack to retrieve a box, after returning to work from a lunch break. Employee #1 was wearing a harness, and the lanyard was attached to the truck, but he was not "hooked off." He walked across a metal slat or skid to reach a box on a rack, which was behind the rack in front of him. On his way back, he fell approximately 17 feet, onto his face. Employee #1 died eight days later.
