Investigation Summary

Investigation Nr: 201345527
Event: 09/20/2010
Employee's Knee Is Fractured Caught in Press

At approximately 2:30 a.m. on September 20, 2010, Employee #1 was assigned to service the Beckhart wastewater filter press. He depressurized the unit and separated the bank of approximately 26 filters to remove the ink cake. Once the filters were cleaned, the system was activated to recompress the filters. The hydraulic ram is activated manually by turning a selector switch located on the control panel at the rear of the unit. The energized ram travels approximately 16-in. and takes approximately 36 seconds to extend and start the compression process on the filters. He was replacing the tape used for sealing the plastic liner onto the unit, where it is generally stored when the Number 1 and Number 2 filters compressed, trapping his left knee, crushing it. Employee's #1 was hospitalized with a fractured knee.
