Investigation Summary
On November 21, 2009, Employee #1 was working on the roof-top of the building, cleaning the air-conditioning unit with a pressure washer. He fell through an unguarded skylight that was 24 feet above the floor. He sustained multiple fractures and was hospitalized for treatment.
