Investigation Summary
At approximately 3:00 p.m. on December 10, 2009, Employee #1 was picking up small pieces of wood and was putting them by a fire. An operator of a track hoe would pick them up and put them in the fire. Employee #1 turned and walked toward the track hoe, but the operator did not see him. The operator swung the arm back, and the bucket teeth struck Employee #1 in the abdomen. He was taken to the hospital and later died.
