Investigation Summary
On October 23, 2009, Employee #1 was unloading five culvert pipes from a semi-trailer. The pipes ranged in length from 28 to 38 feet. As Employee #1 cut the metal banding that secured the pipes together, two of the pipes became separated and fell from the trailer, striking and killing Employee #1.
