Investigation Summary
At approximately 9:15 a.m. on November 20, 2009, Employee #1, a warehouse employee, was operating a Crown Stockpicker. After having retrieved a piece of glass from storage, she struck a storage rack, while driving backwards out of an aisle. She struck the storage racks with such force that stored automobile windshields were dislodged and fell 22 feet off of the top shelf, striking and crushing Employee #1. She died at the scene.
