Investigation Summary
On October 28, 2009, Employee #1 was insulating pipes from a scissor lift at a height of approximately 10 feet when his coworkers noticed that he had collapsed and became unresponsive. Emergency services were contacted and CPR was performed until their arrival. Employee #1 was taken to Centennial Hospital, where he was determined dead.
