Employee's Fingers Are Amputated by Hook Locking Mechanism

At approximately 4:00 p.m. on August 24, 2009, Employee #1 was working with a coworker to remove a gear box from the mold changing table (Model Number ASY1507A, Serial Number 1431) at the injection mold. The gear box was being removed because a shaft was coming out of it. Employee #1 was disassembling the gearbox, and the coworker was operating the controls. In order to remove the gear box, the chain assembly needed to be removed to alleviate some of the weight associated with the gear box removal. They needed to expose the chain so the hook device could be removed from the chain. The machine was then locked out. It was determined that the chain was not exposed far enough and needed to be taken out a few inches further. The lockout was removed, and Employee #1 was resting the gearbox so that the chain would not be stressed. The coworker attempted to extend the chain. The lock forward function actuated, and the locking mechanism in the hook severed Employee #1's middle and ring finger on the left hand. Employee #1 was taken to the University of Iowa by ambulance, where he was hospitalized.
