Investigation Summary
On August 1, 2010, Employee #1 and a coworker were installing the trim and gutter straps on a roof. They were elevated in a shop-made man-basket and had their fall protection (safety harnesses) properly secured to the lifelines in the basket. This was the last work to do to complete this newly constructed commercial building. The employee disengaged his safety harness from the life line and climbed from the basket onto the roof so that he could continue to install the gutter straps from the roof. He could not reach the area where he was working from the basket. While on the roof, the employee did not secure his safety harness to an existing life line on the roof. He fell approximately 20 ft from the roof onto a concrete surface. The employee was taken to a local hospital. Employee #1 died during surgery. The cause of death was blunt trauma to the chest. His injuries included lacerations, contusions and abrasions.
