Employee Is Killed When Steel Coil Falls

At approximately 4:19 p.m. on October 14, 2009, an employee was killed when an 18,730 lbs steel coil fell on him from an Exit Coil Cart. At the time of the accident, the employee current position was a janitor, but he has had various job changes through his 40 years employment with the company and he was not regularly assign to this task. A coworker was in the area that saw the employee preparing the steel coil to be wrapped with paper on the Exit Coil Cart. The coworker saw the employee, began to manually place paper on the steel coil and rotated the Coil Cart so that the paper would wrap around the steel coil. The coworker also noted that the paper did not wrap around the coil correctly. The employee set the Exit Coil Cart in motion for paper wrapping. The coworker saw that the employee tore the wrapping paper off the coil and threw it away in the trash, and he began the wrapping process again. The coworker observed that the employee began to manually place the paper again on the steel coil. At that moment the coworker turned away from the area to do his own job duties. The coworker heard the loud thump and ran over to the Re-Coiler Assembly Area where he saw the steel coil on top of the employee. The coworker immediately called emergency services to have them respond to the accident. The employee was killed.
