Investigation Summary
On March 15, 2010, Employee #1 stacked one pallet of bottled water on top of a second pallet of bottled water. The stacked pallets became unstable when the employee moved them. Shortly after, the stacked pallets toppled onto the employee. Employee #1 died from mechanical asphyxiation.
