Employee Is Killed in Fall from Ladder

Employee #1, a maintenance man, was found on the floor near an unopened stepladder that was leaning against a textile machine. The cause of death was blunt head trauma. The step ladder did not have any apparent defects. The textile machine was not running at the time of the accident and his coworkers did not understand why he went up on the ladder. Ladder training was not required by the employer. The office issued a 5(a)(1) letter to the employer recommending ladder training for all employees.
