Investigation Summary
At approximately 12:35 p.m. on May, 23 2011, Employees #1 and #2 were affixing overhang build-outs along the west gable end of a 54 foot by 72 foot new construction wood-framed storage barn. Employees #1 and #2 worked from inside the trusses that were seated atop the 2 inch by 6 inch walls. No diagonal braces were in place on the trusses, nor were there any cross-braces in place in the truss web planes. The structure shifted in a northeasterly direction and began to collapse. Employee #1 and #2 were sitting atop the collapsing structure and fell with the structure approximately 16 feet to the concrete slab below. Employee #1 fell onto the concrete foundation and was then struck by fallen structural members. He suffered fractures and head injuries. Employee # 2 landed atop the fallen trusses and suffered mild abrasions and contusions. Waynesfield Emergency Services and the Auglaize County Sheriff's office responded to the accident, and the injured employees were transported to the Lima Memorial Hospital in Lima, OH. Both employees were hospitalized for more than one night. Initial evidence indicated inadequate temporary lateral and diagonal bracing supporting trusses and bearing 2 inch by 6 inch walls.
