Investigation Summary
On November 20, 2010, Employee #1 was working at an envelope manufacturing facility. While he was accessing rollers of an envelope production machine to adjust and troubleshoot a malfunction with its cylinder, an outside representative of the machine's manufacturer was using a computer laptop to program the machine. With the machine's guard removed, the machine was activated during the programming process. As the machine parts began moving, Employee #1's right thumb became caught in the machine and was amputated. The accident investigation revealed that the machine was recently installed and no lockout procedures had been developed or implemented.
