Investigation Summary
On November 23, 2010, at approximately 8:30am at Heritage Steel Services, Inc. a male ironworker received fractures of the ribs after falling off a 12 foot straight ladder. The injured employee had climbed the ladder which was supported by a masonry block wall. While standing on the ninth or tenth rung, the employee attempted to toss a synthetic rope over the block wall. At some point during this activity, the ladder shifted and slid parallel against the wall. The employee fell to the concrete pavement below in the opposite direction.
