Investigation Summary

Investigation Nr: 202089470
Event: 11/29/2010
Employee Is Struck and Killed by Truck in Parking Lot

At approximately 2:00 p.m. on November 29, 2010, Employee #1, an English-speaking Caucasian who was an over-the-road driver for a trucking company, had just left the security guard shack at the site of the host employer (a steel tube manufacturer). As Employee #1 was walking across a parking lot to return to his semi-truck, the right front end of a second truck, operated by an over-the-road driver for a different trucking company, struck him. Employee #1 sustained crushing injuries and was killed. The accident event occurred outdoors, in an open parking lot. Both Employee #1 and the other truck driver were working alone. However, two employees of the host employer directly witnessed the incident, which was also recorded by the security camera of the guard shack. There was no precipitation, fog, or ice, and weather was not a factor. No other known factors contributed to the incident.
