Employee's Finger Is Caught by Welder and Is Amputated

At approximately 11:30 p.m. on July 29, 2010, Employee #1 was working at a dishwasher manufacturing facility. A coworker unexpectedly cycled a table welder (Miyachi STA-200A), and the employee's left index finger was pinned between the welder electrode and the weld table. The employee's finger was crushed, burned and amputated. Employee #1 was hospitalized. The accident occurred indoors; therefore, weather was not a factor. There was one witness to the accident.
