Investigation Summary
On September 1, 2009, Employee #1, an electrician, stepped or kicked an electrical junction box with a cover. This junction box had feeds for the building lights and a control switch for the lights in a cooler. This impact of the junction box resulted in a small arc flash turning off the power to the 277 volt lights. It was determined that the circuit remained energized, and Employee #1 still attempted to examine the junction box. Employee #1 was electrocuted.
