Investigation Summary
On November1, 2010, two employees were removing trees from half acre of land. Employee # 1 was working at the rear of the dump truck, and he was struck by a tree loaded on a truck by Employee #2. Employee#1 died on November13, 2010. The cause of death was listed as a fracture of the thoracic spine.
