Investigation Summary
On June 15, 2010 Employee #1 and four other employees were replacing the roof of the building at 21 Stone Castle Rd. in Rock Tavern, NY. A section of steel sheeting, approximately thirty feet in length had been removed, and insulation was exposed. Employee #1 used a crowbar to attempt to remove tar from a roof vent flange, located next to the insulation. Employee #1 stepped on the exposed insulation, fell through the opening, and landed on the concrete floor approximately twenty to twenty-four feet below. At approximately 08:40, emergency services were contacted and found Employee #1 unconscious, but breathing. EMTs attempted resuscitation, but found that Employee #1 had died.
