Employee Crushed by Machine

Employee #1 was operating a Columbia Block machine when he started receiving the fault code indicating that the pallet magazine was empty. Employee #1 checked the magazine and noticed that it was not empty. Employee #1 emptied the magazine, and had coworker place the pallet feed line on manual, as Employee #1 went to get a screw driver to make the adjustments. Employee #1 got in the machine when the metal screw driver contacted the proximity switch, which activated the pallet sled crushing him. Employee A was pronounced dead by EMS at the scene.
